A Journey Home: Transforming Care, Providing Support, Expanding Access

When everyone has a safe, stable place to call home, people are healthier and communities can thrive.

To intervene effectively, we must meet immediate needs, while also addressing underlying causes of housing insecurity and homelessness.

In this forum, participants learned about how health system leaders are joining with others to:

  • leverage Medicaid to provide housing supports and services for people who are currently homeless;
  • link people to services that can prevent homelessness, through legal assistance and other supports; and
  • reduce housing insecurity by expanding access to affordable housing.
